Job Opening Summary

Works under the supervision of UFP leadership. Serves in an expanded nursing role to collaborate via telemedicine outreach with a select group of high-risk patients to optimize medical outcomes. Closely works with patients through recurrent telemedicine outreach to provide a model of care that delivers quality, efficient and cost-effective health care services. Collaborates with the patient's primary care provider and specialists, when applicable, to develop and encourage patient care plans. Assesses, plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient's health status. Integrates evidence-based clinical guidelines, preventive guidelines and protocols in the development of treatment plans. Develops systems of care and creates patient outreach opportunities that monitor patient progress and promote early intervention in acute care situations. Works effectively through telemedicine services with other members of the health care team to optimize interventions.
 

Job Opening Qualifications

Minimum Education and Experience Requirements:

  • Licensed as an RN in Florida.
  • ADN required; BSN preferred.
  • Bachelor's degree in health care preferred.
  • Case manager certification preferred.
  • Three to five years of experience in clinical nursing and case management. Telemedicine experience preferred.
  • Knowledge of the basic concepts and principles of managed care are required.
  • Critical thinking skills and the ability to analyze complex data sets required.
  • General computer knowledge and capability to use computers, specifically telemedicine platforms like Zoom, required.
  • Demonstrates customer-focused interpersonal skills to interact in an effective manner with practitioners, the interdisciplinary health care team, community agencies, patients and families with diverse opinions, values and religious and cultural ideals.
  • Demonstrated ability to work autonomously and be directly accountable for practice.
  • Demonstrated ability to influence and negotiate individual and group decision-making.
  • Demonstrated ability to function effectively in a fluid, dynamic and rapidly changing environment.
  • Demonstrated ability to function effectively in a telemedicine platform of care.
  • Demonstrates leadership qualities, including time management, verbal and written communication skills, listening skills, problem solving and decision-making, priority setting, work delegation and work organization.


Licensure/Certification/Registration:

  • Current licensure by the State of Florida as a registered nurse.
  • Basic Life Support certification or obtained at first opportunity with employment.
  • OCN strongly preferred.


Minimum Pay Rate - Hourly: $25.99
Midpoint Pay Rate - Hourly: $35.80
